Four gates: ① Defects: none; the candidates reviewed were fewer than 200 participants and under 12 weeks. ② The n=53 sample and week-1 endpoint formally match those listed items. ③ The paper randomized 53 patients, 35/18, and measured the primary endpoint "at the end of week 1." ④ Large recruitment is realistically impossible in this rare disease, and the question was rapid control of an acute flare; placebo patients crossed to open-label spesolimab after week 1, making a longer placebo comparison impossible. Both are unavoidable and therefore not counted, giving B0 with no defects.
