Custom functional foot orthoses,
does it really help with Relief of walking pain at six months in mild-to-moderate hallux valgus?

30-Second Summary
B
Evidence Grade B · 70 · Safety acceptable
Custom daytime orthoses reduced pain at six months, but the difference disappeared by one year
No serious harm signal from custom orthoses was reported in the trial. Pressure, friction, skin irritation, or worsening pain should prompt discontinuation and reassessment of fit.
What the
research shows
The grade is B with 70 points. The trial randomized 211 people and analyzed 209 after two exclusions. In that trial, custom orthoses reduced walking pain by an adjusted 14 points on a 100-point scale versus watchful waiting at six months (95% CI 6 to 22). The standardized difference from endpoint standard deviations was about 0.60, a medium statistical effect. At 12 months, the six-point difference was null, so this supports temporary symptom relief rather than lasting correction.
What the
ads claim
The tested device was a cast-made daytime in-shoe functional orthosis. Its result cannot be transferred to retail night correctors or silicone toe spacers.

Useful facts when choosing a product

  • Korean markets sell custom functional insoles, night bunion correctors, and silicone spacers, but the trial tested only cast-made polypropylene custom orthoses.
  • At six months, pain changed 50 to 36 with orthoses and 45 to 45 with waiting; adjusted difference -14 (-22 to -6).
  • At 12 months, pain changed 50 to 40 and 45 to 40; adjusted difference -6 (-15 to 3), not significant.

What the research actually shows

Torkki randomized 211 people and analyzed 209 after two exclusions: surgery 71, orthoses 69, and a one-year untreated waiting list 69. An experienced clinician took negative casts and sent individualized prescriptions; ProLab's role was limited to fabricating and shipping the bilateral polypropylene products. The report does not establish free provision or research funding by ProLab. These were not night splints or prefabricated silicone spacers. Reported use averaged 5.8 hours per day on six days weekly at six months and 5.5 hours on six days weekly at 12 months. Follow-up was 99% and 98%, and analysis followed allocation with last observation carried forward for limited missing data. Data analysts were blinded, but participants and assessors could not be blinded. The control was a one-year untreated waiting list with no sham orthosis, leaving expectation effects for unblinded self-reported pain. Funding came from the Finnish Office for Health Technology Assessment, Finnish Medical Foundation, Scientific Foundation of Jorvi Hospital, and Scientific Foundation of Mehiläinen Hospital. Prospective registration could not be confirmed.

02

Why this is classified as B (70)

The positive trial randomized 211 and analyzed 209, but no sham and unblinded self-reported primary pain create avoidable expectation bias, giving B with 70 points.

Counterpoint. Temporary pain relief does not establish lasting angular correction or durable benefit.

Rejudgment record. Cross-check applied — Three-arm design with verified absolute pain changes, dispersion, adjusted confidence intervals, and separate assessment of six-month pain, 12-month durability, and radiographic angle

Sub-claim grades by effect

This ingredient is marketed for several effects. A single overall grade blends strong and weak claims together, so each effect is graded separately here. The overall grade reflects the strongest disconfirming or core claim.

Effect (sub-claim)GradeBasis
Walking-pain relief at six monthsBAdjusted difference was -14/100, standardized difference about 0.60.
Sustained pain relief at 12 monthsDThe adjusted difference was null, -6 (-15 to 3).
Correction of hallux valgus angle?Radiographs at 12 months were obtained only in the surgery group, so orthosis angular change was unavailable.

Cross-check — AI research and Codex final gate

AI was used for research and drafting. Blind grading, adversarial audit, and the methodology boundary rules were then reapplied before Codex performed the final evidence, grade, copy, and build checks. If a grade cannot be narrowed, both evidence positions and their reasons are published.
