B. coagulans Unique IS-2,
does it really help with Bowel regularity and constipation improvement?
research showsDirect bowel-function RCTs of Unique IS-2 show short-term positive signals, but all are commercially linked and generally last about four weeks. Manufacturer and proprietary-strain concentration and some null final comparisons support C with 55 points.
ads claimMarketing treats spore-forming acid survival as proof of constipation treatment or a larger effect. Survival is a delivery property, not clinical effect size, and findings from other B. coagulans strains cannot automatically be transferred to Unique IS-2.
Useful facts when choosing a product
- In South Korea, Bacillus coagulans Unique IS-2 probiotics are distributed as individually recognized ingredient No. 2022-45, with a daily intake of 2×10^9 CFU.
- The pivotal constipation RCT also used 2×10^9 CFU/day for four weeks. Labels should be checked to determine whether CFU is guaranteed through the end of shelf life.
- Korean guidance advises caution in people with disease or medicines, allergic constitution or short-bowel syndrome, infants, and pregnant or lactating people.
- Spore-survival data describe the likelihood of delivery but do not guarantee the magnitude of a bowel effect or an individual's response.
What the research actually shows
The Madempudi 2020 RCT gave 100 adults with functional constipation Unique IS-2 at 2×10^9 CFU/day or placebo for four weeks and reported improvements in weekly bowel movements, normal stool consistency, difficulty defecating, and pain. Three key authors were employed by Unique Biotech. The Venkataraman 2023 trial randomized 150 adults with constipation to Unique IS-2 plus lactulose, lactulose, or placebo. Stool characteristics and symptoms improved earlier, but week-four bowel frequency did not differ significantly from lactulose, making the strain's isolated effect difficult to determine. The LeMoire 2025 RCT in 144 healthy adults with infrequent bowel movements reported improvements in hard stools and stool characteristics, but gastrointestinal symptoms, quality of life, microbiota, and the proportion of complete spontaneous bowel movements were not significant; the trial was PepsiCo-supported and industry-affiliated.
Why this is classified as C (55)
Direct bowel-function RCTs exist, but all are commercially linked and short term, without independent long-term replication, supporting C with 55 points.
Counterpoint. Adults with functional constipation may improve bowel frequency and stool characteristics after 2×10^9 CFU/day for four weeks. It should not replace standard constipation care before independent replication.
Rejudgment record. Reassessment (cross-check reflected) — Direct bowel-function RCTs exist, but all are commercially linked, generally last four weeks, and lack independent long-term replication

Evidence Table
| Study | Design | Sample | Funding | Endpoint | Result | Weight |
|---|---|---|---|---|---|---|
| Madempudi RS et al. 2020 | Randomized double-blind placebo-controlled trial | 100 | Key authors employed by Unique Biotech | Weekly bowel movements, stool consistency, defecation difficulty, and pain | Several bowel and symptom outcomes improved versus placebo after 2×10^9 CFU/day for four weeks. | Key |
| Venkataraman R et al. 2023 | Randomized double-blind three-arm trial | 150 | Three authors employed by Unique Biotech | Stool frequency, consistency, incomplete evacuation, and pain | Adding the strain accelerated early improvement, but week-four frequency was not significant versus lactulose alone. | Key |
| LeMoire A et al. 2025 | Randomized double-blind placebo-controlled trial | 144 | PepsiCo-supported with industry-affiliated investigators | Bowel movements, stool characteristics, complete spontaneous bowel movements, GI symptoms, quality of life, and microbiota | Hard-stool and stool-characteristic signals were positive, but GI symptoms, quality of life, microbiota, and the complete-spontaneous-bowel-movement proportion were null. | Supportive |
