Modafinil,
does it really help with Improvement of chronic fatigue and fatigue-related daily functioning in people with multiple sclerosis?
research showsModafinil is graded C for multiple-sclerosis fatigue because small positive trials conflict with a larger negative trial. In the 141-participant TRIUMPHANT-MS trial it did not significantly outperform placebo for fatigue or fatigue-related quality of life and caused more adverse events. Earlier small studies and a 2024 meta-analysis found modest signals. Evidence for wakefulness in healthy people or treatment of narcolepsy was not attributed to MS fatigue.
ads claimWakefulness promotion and the narcolepsy indication must not be expanded into recovery of complex chronic MS fatigue and daily function. This use is off-label.
Useful facts when choosing a product
- Modafinil is a prescription wakefulness-promoting drug for excessive sleepiness in specified sleep disorders; use for MS fatigue is off-label.
- Sleep disorders, depression, pain, anemia, infection, and medicines should be assessed as potentially correctable contributors to MS fatigue.
- Headache, anxiety, insomnia, nausea, rare serious skin reactions, misuse or dependence potential, and drug interactions require prescriber review.
What the research actually shows
The 72-participant sequential single-blind Rammohan 2002 study found improvement at 200 mg/day versus the placebo run-in but not at 400 mg/day. In the 141-participant randomized double-blind crossover Nourbakhsh 2021 trial, modafinil, amantadine, and methylphenidate were not superior to placebo. A 2024 meta-analysis reported an MFIS mean difference of -4.42 and a small quality-of-life benefit, but included trials were heterogeneous in size and design.
Why this is classified as C (45)
Credible small positive human trials and a recent meta-analysis provide a modest signal, while a 141-participant crossover trial denied superiority over placebo. The conflicting-large-trial rule and subjective surrogate ceiling yield C with 45 points.
Counterpoint. Evidence does not support indiscriminate prescribing, although a specialist may consider a monitored trial in selected patients, including assessment of daytime sleepiness.
Rejudgment record. Cross-check applied — Applied the conflicting-large-trial rule: accepted early small positive trials and a modest recent meta-analytic signal while incorporating the 141-participant TRIUMPHANT-MS failure to beat placebo and the subjective fatigue and quality-of-life endpoints
Sub-claim grades by effect
This ingredient is marketed for several effects. A single overall grade blends strong and weak claims together, so each effect is graded separately here. The overall grade reflects the strongest disconfirming or core claim.
| Effect (sub-claim) | Grade | Basis |
|---|---|---|
| Reduction of MS-related fatigue | C | Small positive trials and a meta-analysis conflict with a larger negative 141-participant trial. |
| Improvement in fatigue-related daily functioning | C | A small pooled quality-of-life signal was not confirmed by the larger trial. |
| Short-term medication response in MS fatigue | C | Some small trials found a response, but TRIUMPHANT-MS denied superiority over placebo. |

Evidence Table
| Study | Design | Sample | Funding | Endpoint | Result | Weight |
|---|---|---|---|---|---|---|
| Nourbakhsh B et al. 2021 | Randomized placebo-controlled double-blind four-period crossover trial | 136 | Public comparative-effectiveness support from PCORI | MFIS, fatigue-related quality of life, sleepiness, and adverse events | Modafinil was not significantly superior to placebo and adverse events were more frequent. | Key larger negative trial |
| Rammohan KW et al. 2002 | Multicenter single-blind sequential placebo and dose study | 72 | Product trial with reported industry context | Fatigue scales and sleepiness | Fatigue improved at 200 mg/day versus placebo run-in but was not reproduced at 400 mg/day. | Early small positive signal |
| Ghazanfar S et al. 2024 | Systematic review and meta-analysis of controlled clinical trials | 7 | Academic research; no external funding reported | MFIS, ESS, quality of life, and adverse events | The MFIS mean difference was -4.42, a modest positive result, with increased adverse-event risk. | Recent conflicting synthesis |
