A meta-analysis of 19 placebo-controlled studies found a small but significant overall cognitive effect, g=0.10 (95% CI 0.05 to 0.15), and a separate meta-analysis reported SMD 0.12. These are positive surrogate signals, but the effects are very small and come mainly from single-dose laboratory tasks, so they do not support B or guarantee everyday productivity or durable benefit. Wakefulness during sleep deprivation or night work is a separate claim, and evidence does not establish treatment of ordinary fatigue or replacement of sleep. Dependence, insomnia, cardiovascular concerns, and off-label use remain separate safety warnings.

- Provigil is a Schedule IV controlled substance in the United States and has misuse and dependence potential, so it should not be shared or obtained from unregulated nonprescription sources.
- Headache, anxiety, insomnia, and nausea can occur, while blood-pressure or heart-rate changes, psychiatric symptoms, and rare serious skin reactions require caution and prompt evaluation of any rash.
- Modafinil can reduce the effectiveness of steroidal contraceptives, and additional contraception is required during treatment and for the label-specified period after discontinuation.
What the research actually shows
The 2019 meta-analysis by Kredlow and colleagues pooled placebo-controlled single-dose studies in healthy non-sleep-deprived adults and reported an overall cognitive effect of g=0.10 (95% CI 0.05 to 0.15). The 2020 meta-analysis by Roberts and colleagues likewise found a small significant overall effect, SMD 0.12 (P=.01). Battleday and Brem identified executive-function signals alongside mixed domain results and possible impairment of creativity. The 32-person night-work trial by Walsh and colleagues belongs to the separate high-sleep-pressure wakefulness axis and is not combined with normal daytime cognition. Approved sleep-disorder indications do not constitute recognition of a healthy nootropic claim.
