Useful facts when choosing a product
- The paper reported 3.8 g salt per 40 g fresh equivalent of the trial product, converting to about 1,495 mg sodium or 75% of a 2,000 mg daily value. Separately, the 10% to 13% salinity range for traditional doenjang generally gives a background range of about 1,572 to 2,044 mg sodium, or 79% to 102%, per 40 g.
- Shukla et al. reported mean histamine of about 268 mg/kg and mean tyramine of about 1,267 mg/kg across 23 traditional doenjang samples. These were not assays of the trial product; products vary, and people taking monoamine-oxidase inhibitors need particular caution with tyramine.
- Nitrite and N-nitroso compounds in the trial product were not confirmed.
- Records of East Asian fermented pastes are old, but they cannot automatically be treated as records of the same product as modern Korean doenjang.
- A visceral-fat reduction and potentially high sodium exposure can both be true because they are different claims.
What the research actually shows
The tables in Cha YS et al. 2012 were checked directly. Twelve-week visceral-fat change was −8.6 cm² with doenjang and about −0.6 cm² with placebo, and the between-group standard error was 3.7 cm². Conversion using a t distribution with about 49 degrees of freedom gives an approximate confidence interval of 0.5 to 15.3 cm². The covariance of change was unavailable, but the public table allowed direction and approximate standardized magnitude to be calculated.
Why this is classified as C (50)
The calculable effect reached a conventional medium magnitude, but CT visceral fat is an imaging marker rather than a clinical event. Evidence is one small trial analyzing 51 completers without a confirmed preregistration, yielding C with 50 points.
Counterpoint. The publicly funded positive signal merits follow-up, but independent replication including lower-sodium products and long-term clinical outcomes is needed.
