- Activated charcoal can nonspecifically adsorb many prescription medicines, nonprescription medicines, and supplements in the gastrointestinal tract and reduce their absorption.
- Black stools, nausea, vomiting, and constipation can occur, and administration to an intoxicated person with impaired consciousness creates an aspiration risk.
- Taking charcoal does not lower blood alcohol or make driving safe; only elapsed time allows ethanol metabolism to proceed.
What the research actually shows
Minocha 1986 gave six healthy young adults enough ethanol to target a peak concentration of 125 mg/dL and, on one crossover day, administered 60 g of superactivated charcoal first. The absorbed fraction was similar and the average peak concentration was 8% higher with charcoal at p=0.08. In a separate randomized crossover study, Hultén 1986 administered 20 g of charcoal 30 minutes after 88 g of alcohol and found no significant plasma alcohol difference. The 2026 Hoegberg international recommendation used a systematic evidence assessment and reached consensus that activated charcoal has no role in poisoning by ethanol, methanol, or ethylene glycol. No activated-charcoal human randomized trial directly assessing hangover severity or functional recovery was identified.
