Useful facts when choosing a product
- The best-fitting evidence concerns MCT composed mainly of caprylic acid C8 and capric acid C10, which is not equivalent to coconut oil or mixed medium- and long-chain triglycerides.
- Meta-analytic benefits were generally observed when MCT calories replaced long-chain fat, so adding MCT oil to the existing diet can raise total energy intake and offset the effect.
- Large single doses can cause nausea, abdominal discomfort, bloating, and diarrhea, making gradual introduction and divided doses more tolerable.

Weight heterogeneity was 35%, but doses and energy-control methods varied and commercial bias was identified. A contemporaneous analysis by Bueno included 11 trials, only five at low risk of bias, and rated the overall evidence low to moderate. A 2024 analysis in adults with overweight or obesity found a positive weight signal for pure MCT but not for mixed medium- and long-chain triglycerides, emphasizing product composition. Long-term weight maintenance, cardiovascular events, and diabetes complications have not been established by this evidence.

Several randomized-trial meta-analyses repeatedly found a small benefit when MCT replaced LCT, but the absolute loss was small and energy control, duration, product composition, and commercial bias limit confidence, yielding C with 55 points. Gastrointestinal effects and energy density are separate safety and use considerations.
