- This efficacy claim concerns 127 g of fresh whole grapefruit, not juice; fiber and chewing differ between the forms.
- Intestinal CYP3A4 inhibition can raise exposure to particular medicines including simvastatin, atorvastatin, nifedipine, and cyclosporine; it does not apply to every drug.
- Fexofenadine exposure can instead fall because grapefruit inhibits transporters and reduces absorption. These interactions are separate from weight-loss efficacy.
- All groups received calorie restriction, so roughly six kilograms of loss cannot be assigned to grapefruit.

What the research actually shows
Silver and colleagues randomized 85 participants for 12 weeks. The weight outcome matched NCT00581074, but the trial began in March 2006 and was submitted in December 2007. The article did not clearly name a grower organization as the main funder; use of Florida Department of Citrus composition data is not funding.
Why this is classified as D (28)
A positive early study was followed by two null trials and a null pooled estimate. The pooled interval still did not exclude a conventionally small effect, so this was not a precise demonstration of no benefit. The result remains D with 28 points.
Counterpoint. A low-energy preload may help some people follow calorie restriction, but that does not prove a grapefruit-specific effect.
