What the research actually shows
The two direct trials were performed in tropical hospitals in Malawi and Nigeria and analyzed 160 children in total. Agbolosu followed temperature and discomfort for two hours with no reported attrition. Aluka analyzed 80 of 88 randomized children and used cold water. Both predated routine registration or had no verified registration number. Agbolosu funding was not verified, while Aluka stated ‘The research was self-funded’ and ‘Source of Support: Nil’. Small samples, completer analysis in Aluka, and incomplete allocation-concealment reporting were counted as limitations.

Two-hour afebrile status clearly favored acetaminophen, but it is a temperature surrogate and the evidence consists of small, differently implemented trials with completer analysis and incomplete funding and allocation reporting. These constraints give C with 50 points.
