BRACE was a peer-reviewed international multicenter double-blind placebo-controlled phase 3 original report. After 3,988 randomizations, both six-month co-primary outcomes failed in the actual mITT population of 3,386, with five COVID-19 hospitalizations in each group. The peer-reviewed South African phase 3 report randomized 1,000 participants and did not show prevention of COVID-19 hospitalization or infection. The peer-reviewed Dutch BCG-CORONA report randomized 1,511 participants and found null results for its absenteeism primary endpoint and documented COVID-19. Overall protection was null, while positive small studies used weaker definitions or had low precision.

Both symptomatic and severe COVID-19 co-primary outcomes failed in the baseline SARS-CoV-2-negative mITT population of 3,386 among 3,988 BRACE randomized participants, and independent large trials of 1,000 South African, 1,511 Dutch, and 6,112 vulnerable older participants were repeatedly null. Consistency with the simple repeated-null evidence in verdict 1648 gives F with 9 points. This claim is entirely separate from BCG efficacy against tuberculosis.
Counterpoint. Trained immunity remains a legitimate research field, and BCG retains its separate role in tuberculosis prevention. Revaccination for COVID-19 prevention does not replace specific vaccination, ventilation, or treatment for high-risk patients.
